Most insurance plans accepted by Laboratory and Pharmacy. We accept all major credit cards, and ATM card payments via Stripe online payment services.
Annual Membership Special
Available For New Patients
$345 annual cost, which include:
- Annual Conceirge physical assessment (within 10 mile radius..additional mileage will be subjected to additional fees.)
- Completed physical form provided via patient portal
- Follow up tele-med consultation visit to review lab results
- Additional low risk urgent care tele med visit rate $45/per visit
- Patient portal access available
- Electronic prescription to your local pharmacy for non controlled substances, if deemed necessary
Contraceptive Membership
$245 annual cost, which include:
- Conceirge visit (Visit does not include Pap smear, pelvic and vaginal exam)
- Lab work obtained for Thyroid panel, CBC, urine pregnancy test, and STI screening (Patients are responsible for lab bill)
- Follow up tele-med consultation visit to review lab results
- Contraceptive Rx provided in 3 month intervals
- Additional tele-med contraceptive visit and or prescription rate$45/per visit
Urgent Membership
$145 annual cost, which include:
- Easily accessible low risk urgent care tele-med visit rate $45/per visit
- Access to patient portal
- Unlimited communication with provider via patient portal.
- Electronic prescription to your local pharmacy for non controlled substances, if deemed necessary
- Sick or excuse notes provided, if deemed necessary
- Specialist provider referrals, If deemed necessary
**Urgent care visits are for less severe, minor conditions. Examples of these minor conditions include; flu, common cold, ear infections, sinus infections, minor cuts, urinary tract infections, and Sexually transmitted disease symptoms.
**Reasons to go to the ER include chest pain, stroke symptoms, major wounds, and severe burns.
Non Membership Conceirge Physical Exam
$185 per visit, which include:
- In home visit
- In-home lab work as per physical request
- Access to physical assessment via the patient portal
*Concierge visits must be within a 10-mile radius, with additional mileage subjected to additional fees.

Tele-med visit
$135 per visit, which include:
- Provider visit for low risk urgent care complaints
- Portal access for visit summary
- Electronic prescription to your local pharmacy for non controlled substances, if deemed necessary
- If needed, follow up visit $75/per visit
Adolescent Special
*Adolescent refers to ages 13-17 years old
$25 per tele-med visit, which include:
- Sexually transmitted infection education
- Sexually transmitted infection screenings
- Sexually transmitted infection diagnosis
- Sexually transmitted infection treatment
- Contraceptive prescription and management
- Pregnancy test
Medical Marijuana Certification
$135 per certification visit, which include:
- NYS medical marijuana certification provided by a Nurse Practitioner who is certified with the “Medical Marijuana Data Management System.”
Auricular Therapy
Pricing: $135.00
The initial consultation includes an
- Assessment of a single health concern
- Diagnosis of a health concern
- Implementation of the plan of care with an initial application of ear seeds
Each Follow-up Auricular Therapy treatment for initial concern is $55
- Auricular Therapy Concierge service is available to you in the comfort of your home, hotel, or office.
- Initial consultation completed by a Nurse Practitioner, and follow-up service completed by a Registered Nurse
- Concierge Fee $100 (additional to the Auricular Therapy price)
- Concierge visits must be within a 10-mile radius, with additional mileage subjected to additional fees.

Cancelation Policy
We Value your business and ask that you respect our business scheduling policies.
Please notify us at least 24 hours in advance of any cancellation. Any cancellations with less than 24 hours of notice are subject to a cancellation fee amounting to the cost of the scheduled service.
Mental Health Services will be charged $150* for missed appointments that are not canceled 24 hours in advance (i.e. for Monday appointments, you must cancel by the appropriate time the preceding Sunday).
Medication & Therapy Late Fee for clients more than 10 minutes late for their 30-minute session, can still be seen for the remaining time but will incur a $60 late fee, in addition to their insurance or payment responsibility.
In the case that you are more than 15 minutes late and or do not Show up to your scheduled appointment, you are subject to the full no show fee of $150. Alma
